What You'll Need to Enroll
Information to Have Ready:
- Employee Navigator login credentials
- Social Security numbers for you and your dependents
- Birth dates for all family members to be covered
- Current address and contact information
- Beneficiary information for life insurance
Decision Checklist:
- Medical plan (Base, Mid or High plan)
- Check prescription coverage and providers are in network
- Dental and vision coverage
- Supplemental life insurance amounts
- Voluntary benefit selections
Important Reminders
Deadline is Firm
Elections must be submitted by December 14, 2025 at 11:59 PM. Late enrollments cannot be accepted.
No Mid-Year Changes
You cannot change your benefits during the year unless you experience a qualifying life event.
Review Carefully
Double-check all your elections before submitting. You'll receive a confirmation email with your selections.
New Hires
If you are hired during the new year, you are eligible for coverage the 1st of the month following 60 days after your date of hire.
Qualifying Life Events
You can make changes to your benefits mid-year if you experience one of these events
You must notify FBP Advocacy within 30 days of the life event to make benefit changes.
